Hospice
Our Team provides this care in the patient's home, nursing home, hospital and other care facilities or wherever an individual calls home. Our staff is available 24 hours a day, seven days a week. We also offer medications, equipment and supplies that are related to the hospice diagnosis.

Zion's way hospice coordinates a team of freindly experienced professionals to help you with your physical, emotional and spiritual needs.
The team consists of:
- Your Personal Physician
- Hospice Medical Director
- Case Manager/Nurse
- Social Worker
- Certified Nursing Assistant
- Volunteers
- Spiritual Care Counselors
- Breavement Coordinator
- Dietician
Hospice is fully funded through the Medical Part A benefit, for those with a six month or less life expectancy. The hospice benefit is also covered by medicaid and many private insurance agencies
